I use this calender with great satisfaction but i like to show only the months april-may-june-july-august and september. How can i make that possible?

Can someone tell me where and how i can put a direct link to my bookingform? See screenshot Keyframe right BOOK DATE. It has to be placed direct under the link: Availibilty Calendar.
